Required Documents

Documents will be used to verify financial and housing status by a Disaster Case Manager.  Below is a list of the information to be collected.  Please click on the “List of Required Documents” link at the bottom of the page for a specific list before beginning to collect information.

Required Information:

  • Identification

  • Homeownership

  • Assistance received

  • Initial Documentation Verification

  • RREM Documentation

  • Income / Cash

  • Disaster related construction expenditures
